MEDICAL RECORDS
Medical records are the property of Hanson Clinic / Southern Colorado Clinic, PC and it is our responsibility to keep them safe. We utilize both EMR (Electronic Medical Records) and paper charts. Copies of medical records are never released by phone or verbal authorization.

Signed authorization is always required by law, even when releasing records to a patient. This is for your protection as well as ours.
